VIRTUAL CHECK-IN / CHECK-OUT: We are now using a virtual check-in and check-out process. No more clipboards / forms to fill out in the office. You will be emailed or sent a text with a link to fill out your intake info. This will allow for a completely contact-less experience. No more pen and paper! To enable a safer check-out process, we will be requiring that you provide a credit card to leave on file with our office. This will allow us to process your transactions without needing to physically swipe any cards in the office.

PLEASE BE ON TIME: In order to maintain social distancing, spacing between appointments is very important. Late arrivals may be asked to reschedule.

CALL UPON ARRIVAL / WAIT IN YOUR CAR: Please call the office where your appointment is scheduled as soon as you park. Bridgewater Office – (908) 704-8855 ; Rockaway Office – (973) 366-9622Do not enter the office until we instruct you to do so. We will call or text you when we are ready to start your visit. If you have not completed your intake forms, upon arrival we will request that you complete them while waiting in your car. Our staff can email or text the intake forms right to your phone.

FACE MASKS REQUIRED FOR ENTRY: For the entirety of your visit, we will be requiring you to wear a face mask covering your nose and mouth. Our staff will do the same.

ONLY BRING YOURSELF: Please leave any company with you in the car. Patients under 18 years old must be accompanied by ONE parent or guardian. Patients that need assistance with mobility or communication may also be accompanied by ONE companion. If you MUST bring anyone inside, they will be COVID screened and please be sure they are also wearing a face mask covering their nose and mouth and that they keep more than 6 feet from anyone else.

WHEN YOU ENTER THE OFFICE: Please wait at the greeting sign in front of our office before you approach the front desk. Use the hand sanitizer that is provided for you to wash your hands. We will take your temperature with a touchless device to ensure it is not elevated. If we measure an elevated temperature (100.4 degrees F or higher), we will have to ask that you postpone your visit for at least 2 weeks.
